The intent of the Social Service Department is to provide medically related Social Services to maintain the highest practicable physical, mental, or psychosocial well-being of each resident. From the social history obtained at admission, and assessing the resident routinely, it is our goal to help the resident achieve his/her highest level of quality of life.

Social Services is responsible for assuring that the psychosocial needs of a resident are met as placement in long term care is often difficult. Psychosocial programming addresses life style change, adjustment to facility life and on going placement difficulties.

When it is possible for a resident to return home, social service staff are there to assist with discharge planning.
